I hereby consent to participation in interviews, the use of quotes, and the taking of photographs or videos of the child named above by the Dreamers Fellowship Project staff or contracted marketing personnel for the sole purpose of promoting our work with youth in the community.

I also grant to Dreamers Fellowship Project and its agents, the right to edit, use, and reuse said products for non-profit purposes including use in print, on the internet, and all other form of media.

I also hereby release Dreamers Fellowship Project and its agent and employees from all claims, demands, and liabilities whatsoever in connection with the above.

To ensure that the Dreamers Fellowship Project is a positive and enjoyable experience for all participants, it is necessary to establish high standards of behavior. I will uphold the following conduct and behavior standards:

I will be courteous and respectful towards others.
I agree to value and respect ideas of others, regardless whether they are the same as my own or not.
I will dress appropriately at all times. I will not wear revealing clothing or apparel featuring profanity, sexual content, alcohol, tobacco or drug related messages or images. The program coordinators reserve the right to assess the meaning of appropriate clothing when deemed necessary.
I will not use any alcohol, tobacco or other drugs or engage in any sexual behavior or violent outburst at any time during program.
I understand that I will forfeit my position in the Dreamers Fellowship Project for misconduct or repeated negative behavior that is unbecoming of a Dreamers Leader.
As a representative of Dreamers Fellowship Project, I represent not only myself, but the members throughout my community. I pledge to uphold this commitment, in addition I understand that if I am not able to remain in good standing during the session I will be asked to leave.

Participação Essencial: Reconheço e compreendo que a participação dos pais/responsáveis é de suma importância para o sucesso acadêmico e pessoal dos alunos envolvidos no Programa.
Obrigações: Comprometo-me a participar ativamente de no mínimo três atividades ao longo de todo o período de duração do Programa, conforme orientado pela equipe organizadora.
Responsabilidade: Assumo a responsabilidade de estar presente e envolver-me nas atividades propostas, contribuindo para o desenvolvimento integral dos alunos participantes.
Conscientização: Estou ciente de que minha presença e apoio são fundamentais para promover um ambiente de aprendizagem positivo e encorajador para os alunos.
Compromisso: Comprometo-me a estar disponível e engajado nas atividades propostas, buscando sempre incentivar e apoiar o progresso acadêmico e pessoal dos estudantes.
Compreensão: Entendo que o não cumprimento das obrigações estipuladas neste acordo pode comprometer o sucesso do aluno no Programa e, consequentemente, sua trajetória educacional e profissional.

Ao assinar este documento, declaro estar ciente e concordar integralmente com os termos acima mencionados, comprometendo-me a cumprir com as minhas responsabilidades enquanto pai/responsável de um aluno participante do Programa Dreamers Fellowship.
